Pilgrims from Khyber Pakhtunkhwa usually fly from Peshawar or Islamabad. Peshawar's Bacha Khan International Airport had non-stop Jeddah flights on Saudia and PIA when we checked, but no Madinah flights. Islamabad has more airlines and non-stop Madinah flights, and is often closer for Hazara. Dera Ismail Khan families often compare Multan. Allow extra road time in winter for mountain areas.
Khyber Pakhtunkhwa sends large numbers of pilgrims to Makkah and Madinah every year, from the Peshawar valley, from Mardan and Swabi, from the mountains of Swat and Hazara, and from the southern districts around Kohat and Dera Ismail Khan. Umrah is the same for all of them. The journey to the airport is not.
A family in Peshawar city can reach the airport in under an hour. A family in the upper Swat valley or the far side of Mansehra may face a long mountain drive before reaching a motorway. A family in Dera Ismail Khan sits almost as close to Multan as to Peshawar. Choosing the right airport, and timing the road journey, is the most important regional decision for KPK pilgrims.

Peshawar families have a local airport with Jeddah flights. When the dates match, it's the easiest option. When they don't, Islamabad is reachable by the M-1 motorway. See Umrah from Peshawar.
These central valley towns are close to Peshawar airport, and most also have good access to the M-1 towards Islamabad. It makes sense to compare both airports for your dates. See Umrah from Mardan.
The Swat Motorway has shortened journeys to the M-1, but the valley roads before it still take time, especially in winter or after rain. Families from Swat usually fly from Islamabad and travel the day before. See Umrah from Mingora.
Hazara is naturally linked to Islamabad through the Hazara Motorway. Islamabad is the clear first choice for most Hazara families. See Umrah from Abbottabad.
Kohat families usually fly from Peshawar, with Islamabad as the alternative for more flight choice. See Umrah from Kohat.
D.I. Khan is far from Peshawar, and often closer in practice to Multan, which has both Jeddah and Madinah flights. Islamabad is the other option. See Umrah from Dera Ismail Khan.

Many families, especially those with elderly parents, prefer to begin in Madinah. You land in the calmer city, rest, pray in Masjid an-Nabawi and visit the Rawdah. You don't wear Ihram on the flight. Later you put on Ihram at the Madinah Miqat (Dhul Hulayfah) on the road to Makkah and arrive for Umrah rested.
Because we found no Madinah flights from Peshawar, KPK families wanting this option usually fly from Islamabad, or from Multan for the south. Alternatively, you can fly Peshawar–Jeddah and take the road or the Haramain train to Madinah first, then return to Makkah. That adds travel, but lets you start with Madinah. Ask for both versions priced.
Because Peshawar flights land in Jeddah, most KPK pilgrims start with the Umrah itself. Knowing how that first day usually goes helps you plan:

For many KPK families, the language of religious guidance matters a great deal. Elders may understand the rites best in Pashto, and Hazara families may prefer Hindko. When booking:
Saudi rules have relaxed for adult women travelling in organised groups, but document checks and operator policies vary. Many KPK families prefer women to travel with close male relatives in the same group. Whatever your family's arrangement, tell the operator about every woman travelling before booking, and ask for rooms with family members.
Pilgrims from Swat, Hazara and other cool areas are often surprised by Makkah's heat, even in winter. Pack light cotton clothes, an umbrella and sandals you've already worn in. Drink Zamzam regularly and avoid midday walks.
In many parts of KPK, Umrah groups form around a village, a mosque or an extended family. These groups can be large and multi-generational. To keep them running smoothly:

A useful middle path: a closer hotel in Makkah, where distance matters most, and a 3 star hotel in Madinah, where many hotels are close to Masjid an-Nabawi anyway.

For the Peshawar valley, the local airport is easiest when it has your date. When it doesn't, Islamabad often does.
A night near Islamabad airport is cheaper than a missed flight.
Multan's Jeddah and Madinah flights can save a long northward journey.
A Pashto or Hindko-speaking group leader can make a real difference for elders' understanding of the rites.
Mountain families should plan hydration, shade and rest in Makkah more carefully than they might expect.
One landslide or snowfall can mean missing the flight. Travel early.
Check first. If you want Madinah first, plan for Islamabad or a road transfer from Jeddah.
Different transliterations of Pashto names are a common source of visa delays. Match CNIC and passport before booking.
New passports take time, especially in peak seasons. Apply as soon as the trip is decided.
